HBO Medicine addresses this challenge by exposing patients to 100% oxygen in a pressurized chamber, typically at 2.0 to 2.5 times the normal atmospheric pressure. Under these conditions, the amount of oxygen dissolved in the plasma increases exponentially, allowing it to diffuse more effectively into tissues.  This influx of oxygen supports a variety of cellular processes that contribute to wound healing. The therapy also enhances the killing power of white blood cells, promotes the release of growth factors, and stimulates the formation of new capillaries in the wound area.

Clinical research has shown that HBO Medicine improves healing in patients with diabetic food ulcers, particularly those classified as Wagner grade III or higher, which indicates deep wounds with bone involvement or infection.  A randomized controlled trial published in Diabetes Care demonstrated that patients receiving HBO Medicine had significantly higher rates of diabetic wound healing compared to those receiving standard wound car alone. Another study, published in the Journal of Wound Care, found that patients undergoing HBO Medicine experienced a reduction in amputation rates and faster wound closure than the control group.  These findings have been supported by systematic reviews and meta-analyses that conclude HBO Medicine is an effective adjunct therapy for patients with non-healing diabetic foot ulcers.

The FDA recognizes the value of HBO Medicine in treating diabetic foot wounds and has approved its use specifically for wounds that are classified as chronic and non-healing.  The therapy is included in the list of approved medical indications under the FDA’s oversight of hyperbaric chamber devices.  The Undersea and Hyperbaric Medical Society (UHMS), a key authority in the field, has also endorsed HBO Medicine for this use, further reinforcing its evidence-based status.  Additionally, the Centers for Medicare & Medicaid Services (CMS) provide reimbursement for HBO Medicine when it is used to treat diabetic food ulcers that have not responded to at least 30 days of standard wound care.

HBO Medicine can help these conditions in many ways.  It helps reduce inflammation and edema, which are common in chronic wounds.  The high levels of oxygen enhance fibroblast activity and collagen formation, which are critical for tissue regeneration.  HBO Medicine also supports the mobilization of stem cells from the bone marrow to the site of the injury, facilitating tissue repair. Also, in the presence of certain bacteria, particularly anaerobic organisms, HBO Medicine has a bactericidal effect that aids in infection control without relying solely on antibiotics.

Despite its benefits, HBO Medicine is not a first-line treatment and is typically used in conjunction with other wound care practices such as infection control, debridement, offloading and optimal management of blood glucose levels. Its use is generally reserved for patients who have not responded adequately to conventional treatment methods.  Each HBO Medicine session can last between 60 and 90 minutes and is administered five days a week for several weeks, depending on the severity of the wound and the patient’s response to therapy.

While generally considered safe, HBO Medicine is not without risks.  Potential side effects include barotrauma to the ears or sinuses, temporary changes in vision, and in rare cases, oxygen toxicity.  However these risks are minimal when the therapy is administered under medical supervision.  Patients are carefully screened before treatment and protocols are in place to minimize complications.

In addition to its clinical benefits, HBO Medicine may also offer economic advantages by reducing the need for amputations and long-term wound care.  A study published in Undersea and Hyperbaric Medicine found that HBO Medicine was cost-effective in preventing major amputations among patients with advanced diabetic foot ulcers. The therapy may be considered expensive for some, but the overall savings from avoided hospitalizations, surgeries, and rehabilitation are significant.

In summary, HBO Medicine is a scientifically validated, FDA-approved treatment for chronic diabetic foot ulcers that have not responded to standard care.
